In need of a change where to start?

My name is Evelyn im 5'2' 375lbs and im in need of a life change but just dont know where to start. I live in Connecticut & im 27 just had a newborn & my health is going down hill. So ive thought about gastric bypass but theres no way i can pay for it on my own so that leaves me at a lost!!!
