Well I went through hell and high water, but my time has come.  My surgery is scheduled for next Thursday and I can't wait. Yippieeeee!  The crazy thing is my insurance approved it over the phone today in like 2 mintues.  I called my surgeon's office at 915 to let them know I was done with my dietary visits and they can submit the info to my insurance company, I got a call an hour later saying that my insurance approved me!!! God is good.  My new life can begin.  Here we go.
