Perry, my surgeons office took care of the paperwork once I filled out the medical stuff and signed a bunch of release forms (so they could get all my info together from All the Dr visits and hospital visits I had over the last 5 yrs), It took them about a week to get it all together then submit it to the insurance. I got my approval 3 weeks from my first consult date. Pretty quick. Just remember if you were treated for any ache, pain or illness to get that documentation. Go back as far as you can and also list any diets (both Dr supervised or not). Good luck. Jamie
